About the Role

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our community services team. As an RN in community services, you will play a crucial role in providing clinical treatment services to targeted populations, including individuals with serious mental illness and/or drug/alcohol addiction. The primary focus of this role is to promote wellness, prevent illness, and improve health outcomes through education, advocacy, and direct patient care.

Majority (90%) of the work will be conducted in the community. Requires extensive knowledge of Medicaid and Department of Behavioral Health and Developmental Services policies and regulatory guidelines.

Responsibilities

  • Deliver, observe, and educate persons served on medication(s), side effects, and use
  • Provide outreach, case management, counseling, advocacy, and other needed services to clients in various environments, including streets, shelters, jails, hospitals, homes, apartments, personal care homes, state hospitals, inpatient hospitals, and rehab settings
  • Collaborate with the treatment team to support consumer-first techniques built on trust and respect
  • Develop treatment plans based on consumers' stage of change for identified problems and needs
  • Implement stage-wise treatment interventions and exercise sound clinical judgment in routine, urgent, and emergent clinical situations
  • Link individuals to essential services and resources as clinically indicated
  • Proficiency in computer documentation and electronic health record systems

Requirements

  • Working knowledge and understanding of medication used in treating psychiatric disorders
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal skills
  • Flexibility to adapt to changing environments and persons served needs
  • BSN in nursing and 1 year of nursing experience OR RN license and 2 years of nursing experience (medical experience a plus)
  • Valid Pennsylvania Registered Nurse license
  • Valid driver license and ability to drive an organization vehicle
  • Act 33/34/74 Clearances (Criminal/Child/FBI Department of Human Services)
  • Pre-employment drug screen
  • Child Protective Services Law training (CPSL)

Schedule

This is an exempt (salary) position, primarily Monday through Friday during daylight hours. Will rotate on-call responsibilities ($50.00 day).
