Registered Nurse - Assertive Community Treatment

Requirements

Graduate of an accredited nursing school and possess active authorization to practice as a Registered Nurse in Pennsylvania.

Valid PA driver's license.

Responsibilities

• Provide direct care in clients' homes and community settings, conducting health assessments, monitoring medication adherence, and addressing medical concerns.
• Oversee the medication system, document treatments, educate clients and team members, and ensure compliance with prescribed regimens.
• Work closely with medical providers, behavioral health professionals, and community resources to ensure integrated, comprehensive client care.
• Participate in crisis response, support treatment planning, and provide health and wellness education to clients, their supports, and the ACT team.

Schedule

Frequent travel (50-90%) is required, and personal vehicle use for client transportation may be necessary.
