How You'll Contribute
- Performs plan-of-care interventions, including medication administration, specimen collection, clinical treatments, and components of Annual Wellness Visits (AWVs)
- Accurately documents all patient care and AWV assessments in the electronic health record
- Maintains accurate documentation of patient condition and preventive-care needs to the care team, physician, patient, and family
- Assists patients with activities of daily living, including hygiene, elimination, nutrition, and ambulation as needed in the clinic setting
- Collaborates across disciplines to coordinate patient care, including referrals, follow-up appointments, preventive screenings, and care-management services
- Evaluates learning needs of patients and families and provides education related to wellness, chronic-disease prevention, and AWV findings
- Advocates for the physical and emotional well-being of patients while supporting long-term health and preventive-care goals

What We're Looking For
- Applicants should have: A current North Carolina or compact state RN license
- Basic Life Support (BLS) certification (required within 30 days of hire)
- Associate's Degree in Nursing required; Bachelor's Degree preferred
- One year of RN experience in a healthcare setting preferred
- Strong critical thinking skills, decisive judgment, and the ability to work with minimal supervision
- Ability to work effectively in a fast-paced or stressful environment
