Registered Nurse Case Manager - RN

About the role

The Registered Nurse (RN) Case Manager collaborates within an interdisciplinary team to coordinate patient care across the continuum, ensuring appropriate utilization management, resource use, and clinical documentation.

Responsibilities

• Assess, plan, implement, evaluate, and communicate patient care plans based on professional standards.
• Lead care coordination efforts, advocating for patients and families, and facilitating communication among providers, external agencies, and team members.
• Provide education on case management and utilization concepts, contribute to committees, research, and community outreach.
• Ensure high-quality, cost-effective, timely care aligned with clinical, financial, and patient satisfaction goals.

Qualifications & Skills

• Graduate of a nursing program with valid RN licensure.
• Preferred: 3 years of acute nursing experience, 1 year in case management, and certifications like CCM or ACM‑RN.
• Current BLS certification from AHA or Red Cross; additional state licenses or certifications as required.
