Registered Nurse Case Manager - RN

Role: The Registered Nurse (RN) Case Manager collaborates within an interdisciplinary team to coordinate patient care across the continuum, focusing on utilization management, resource allocation, and clinical documentation. Responsibilities: Facilitate care planning, ensure high-quality and cost-effective care, advocate for patients and families, and lead educational initiatives on case and utilization management. Act as a communication bridge among providers, patients, families, and external agencies, adhering to professional standards and ethical practices. Qualifications: Graduate of an accredited nursing program with current RN licensure. Preferably with 3+ years of acute care nursing and 1+ year in case management. Certification (CCM or ACM-RN) is preferred. BLS certification is required, along with any additional state licensure or specialty certifications as needed. Skills & Attributes: Strong communication, leadership, and advocacy skills; knowledge of standards from ANA and case management organizations; ability to coordinate complex care plans efficiently. Additional Details: This is a full-time, exempt role with a schedule of weekdays, every 3rd weekend, and holiday coverage.
