About the role
Coordinate the overall interdisciplinary plan of care for patients, from admission to discharge. Create a plan for care across the continuum, integrating patient/family preferences and values. Monitor patient care through assessments, evaluations, and/or patient records. Advocate for resources and remove barriers. Maintain ongoing dialogue with supervisor and other healthcare providers to ensure effective implementation of the health plan.
Responsibilities
- Create and implement the interdisciplinary plan of care for patients.
- Maintain ongoing communication with healthcare providers and stakeholders to ensure seamless care coordination.
- Monitor patient progress and adjust the care plan as necessary.
- Advocate for patient needs and resources to facilitate smooth transitions of care.
- Collaborate with interdisciplinary team members to ensure comprehensive care plans are developed and implemented.
Requirements
- Licensed Registered Nurse (RN) from the Florida Board of Nursing, obtained prior to hire date or job transfer date.
- Certified as a Registered Nurse in Florida.
- Current license to practice in a "Compact State" if applicable.
- BLS Provider certification preferred.
Qualifications
- Diploma from an accredited school/college of nursing.
- Required professional licensure at the time of hire.
Skills
- Strong communication and interpersonal skills.
- Ability to work effectively in a multidisciplinary team setting.
- Proficiency in electronic health record systems.
- Knowledge of hospital policies and procedures.

Schedule
This position is available on a Day Shift basis, with potential for PRN (as needed) coverage.
