Registered Nurse Case Manager Inpatient Rehabilitation

About the role

Coordinate the overall interdisciplinary plan of care for patients, from admission to discharge. Create a plan for care across the continuum, integrating patient/family preferences and values. Monitor patient care through assessments, evaluations, and/or patient records. Advocate for resources and remove barriers. Maintain ongoing dialogue with supervisor and other healthcare providers to ensure effective implementation of the health plan.

Responsibilities

• Create and implement the interdisciplinary plan of care for patients.
• Maintain ongoing communication with healthcare providers and stakeholders to ensure seamless care coordination.
• Monitor patient progress and adjust the care plan as necessary.
• Advocate for patient needs and resources to facilitate smooth transitions of care.
• Collaborate with interdisciplinary team members to ensure comprehensive care plans are developed and implemented.

Requirements

• Licensed Registered Nurse (RN) from the Florida Board of Nursing, obtained prior to hire date or job transfer date.
• Certified as a Registered Nurse in Florida.
• Current license to practice in a "Compact State" if applicable.
• BLS Provider certification preferred.

Qualifications

• Diploma from an accredited school/college of nursing.
• Required professional licensure at the time of hire.

Skills

• Strong communication and interpersonal skills.
• Ability to work effectively in a multidisciplinary team setting.
• Proficiency in electronic health record systems.
• Knowledge of hospital policies and procedures.

Schedule

This position is available on a Day Shift basis, with potential for PRN (as needed) coverage.
