Registered Nurse - Case Manager (Hybrid WFH)

About the role

The Registered Nurse (RN) Case Manager is a vital member of a cross-functional care team, dedicated to advocating for members facing complex health challenges.

Responsibilities

• Identify members needing case or chronic condition management using screening criteria.
• Apply nursing processes to coordinate services and improve health outcomes.
• Educate and guide members and providers to navigate healthcare complexities.
• Ensure high-quality care by adhering to standards, tracking outcomes, and making care adjustments.
• Document activities accurately in databases.
• Maintain effective communication and collaborative relationships with healthcare teams.
• Assist with wellness activities, benefits, and behavioral incentives.
• Perform additional duties as assigned.

Requirements

• Active, unrestricted RN license; multi-state license preferred.
• CCM certification strongly preferred; must obtain within 24 months if not certified.
• Bachelor of Science in Nursing (preferred).
• At least 2 years of clinical experience; 5+ years preferred.
• Strong communication skills, proficiency with Microsoft Office and telehealth tools.
• Trustworthy, accountable, and capable of handling confidential information.
