About the Role
Start date: ASAP. This is a floating position with no fixed ratios; expected ratio is 1:20. Weekend rotation required as needed to meet department needs.
Requirements
- Minimum of 2 years of experience in acute care hospital case management, focusing on care coordination and discharge planning
- Hospital experience is essential
- First-time travelers accepted with clinical screening
- Active California state license required (pending license accepted but must be licensed by start date)
- No more than 7 days of RTO (Return to Office) restrictions
