Registered Nurse/Case Manager

About the role

Coordinates utilization management, resource management, discharge planning, post‑acute care referrals, and care facilitation. Oversees acute patient populations across the care continuum to provide coordinated and integrated care, preventing unnecessary admissions or readmissions. Provides discharge planning coordination and intervention for high‑risk patients in the Emergency Department. Aims to promote patient wellness, improve care outcomes, utilize health services efficiently, and minimize payment denials for patients with complex health needs.

Responsibilities

• Manage utilization and resource allocation across the care continuum.

• Facilitate care coordination to prevent unnecessary admissions or readmissions.
• Provide discharge planning coordination and intervention for high‑risk patients in the Emergency Department.
• Promote patient wellness and improve care outcomes.
• Utilize health services efficiently and work to minimize payment denials.

Requirements

• BLS Certification
• California License
• Minimum 2 years of relevant experience

Schedule

• 5 × 8‑hour shifts, 08:30 – 17:00
• Minimum two weekend shifts per month
• If floated, work will be limited to case management (CM) assignments only
