Responsibilities

• Coordinate and deliver quality patient care as an RN Case Manager.
• Utilize clinical expertise and compassionate approach to assess patient needs.
• Develop personalized care plans.
• Ensure seamless transitions across healthcare settings.

Requirements

• Current NC license as a Registered Nurse issued by the North Carolina Board of Nursing.
• Current CPR certification issued by the American Heart Association (AHA).
