Registered Nurse Case Manager

At Merakey, we put heart and soul into everything we do. We are seeking a compassionate Visiting Registered Nurse (RN) Case Manager to join our team in Warminster, PA, supporting individuals with physical and developmental disabilities.

Requirements

• Graduation from an accredited Registered Nurse program.
• Current license as an RN in the Commonwealth of PA.
• Preferably, at least one year of experience as an RN.
• Genuine interest in working with individuals with physical and developmental disabilities.
• Possession of a current and valid PA driver’s license, along with reliable transportation.
• Ability to perform professional nursing duties including assessment, treatment, and medication dispensing.
• Physical capability to exert up to 50 lbs occasionally, and/or up to 20 lbs frequently, and/or up to 10 lbs continuously.

Responsibilities

• Administer medications and treatments with precision and compassion.
• Perform nursing assessments on residents as needed to identify and address health issues promptly.
• Collaborate with various healthcare professionals and specialists to coordinate comprehensive care, keeping the nurse manager informed.
• Provide health education to residents and their families, including implementing self-medication programs as directed by the nurse manager.
• Conduct in-service training for staff to ensure compliance with protocols and procedures, promoting a culture of excellence in care delivery.
• Communicate health issues with the interdisciplinary team, actively participating in staff meetings.
• Complete quarterly reports maintaining accurate documentation.
• Perform other duties as assigned, contributing to the smooth operation of our program.
• Travel to group homes in Bucks and Montgomery Counties.

Pay

$43/hour + additional $2/hour enhanced pay eligible.

Schedule

Monday – Friday, 9:00 AM – 5:00 PM (in-office plus afternoon travel).
