Certification Requirements
- CA RN License
- BLS
- 5 years of RN Case Management experience
- Inpatient Acute Case Management (Hospital experience required)
Responsibilities
- Coordinate care and transition planning throughout the patient experience.
- Collaborate with physicians, social workers, and RNs to ensure timely patient progression and transition to appropriate care levels.
- Prevent unnecessary admissions or readmissions by facilitating effective resource coordination.
Job Accountabilities
- Conduct patient assessments and develop care coordination plans.
- Monitor utilization management and ensure documentation supports level of care.
- Formulate and communicate transition plans with patients, families, and care teams.
- Identify and address barriers to care progression and transition.
- Engage in department operations and contribute to cost-effective practices.
