Registered Nurse Case Manager
About the Role
We are seeking a compassionate and dedicated Registered Nurse (RN) Case Manager to provide exceptional hospice care to patients facing end-of-life illnesses throughout the Springfield, Missouri region. This role manages a patient caseload, coordinates care with the interdisciplinary team, conducts patient assessments, and ensures patients and their families receive the highest quality hospice services. The ideal candidate is a skilled clinician with strong communication abilities and a passion for delivering patient-centered care.
Schedule
Monday through Friday, 8:00 AM to 4:30 PM (8 hours per day, 5 days per week).
- Weekday On-Call: 4:30 PM to 8:00 AM, 3 days per week
- Weekend On-Call: Saturday 12:00 AM through Monday 8:00 AM, 2 weekends per month
Coverage Area: Within 50 miles of Springfield, MO. Mileage reimbursement provided between patient visits.
Pay
$51.00 - $53.00 per hour.
Responsibilities
- Serve as the primary RN Case Manager for an assigned hospice patient caseload.
- Provide skilled nursing care and symptom management for hospice patients in home and residential settings.
- Perform comprehensive patient assessments and ongoing evaluations of physical, emotional, and psychosocial needs.
- Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary team.
- Coordinate patient care with social workers, chaplains, hospice aides, physicians, and other healthcare professionals.
- Participate in Interdisciplinary Group (IDG) meetings every other Wednesday to review patient progress and care plans.
- Educate patients and family members regarding disease progression, symptom management, medications, and end-of-life care.
- Maintain accurate, timely, and compliant clinical documentation.
- Respond to patient and family needs during assigned on-call rotations.
- Advocate for patient comfort, dignity, and quality of life throughout the hospice journey.
Requirements
- Active Registered Nurse (RN) license.
- Current Basic Life Support (BLS) certification.
- Strong clinical assessment and patient care skills.
- Excellent communication, interpersonal, and compassionate bedside manner.
- Valid driver’s license, reliable transportation, and current auto insurance.
- Ability to manage a field-based caseload and travel throughout the assigned territory.
Preferred Qualifications
- Previous hospice nursing experience.
- Experience using Homecare Homebase (HCHB) documentation software.
- Case management experience in hospice, home health, or palliative care settings.
- Strong organizational and time-management skills.
- Ability to collaborate effectively within an interdisciplinary care team.
