Role Overview

The Registered Nurse Case Manager is responsible for delivering comprehensive hospice care, developing and implementing individualized Plans of Care in collaboration with patients, families, and healthcare team members. This role involves conducting initial patient evaluations, reassessing needs regularly, and coordinating care strategies that address physical, psychosocial, emotional, and spiritual well-being. The nurse documents patient progress, collaborates with the interdisciplinary team, and educates families on self-care techniques, ensuring quality and compassionate care in diverse home settings.

Key Responsibilities

  • Perform initial eligibility assessments within 48 hours of referral
  • Identify and reassess patient and family needs every 15 days or as required
  • Develop, initiate, and coordinate personalized care plans
  • Document clinical information, goals, interventions, and responses
  • Collaborate with patients, families, physicians, and team members for ongoing care
  • Provide patient and family education on self-care practices
  • Maintain accurate clinical notes and adapt care to diverse home environments

Qualifications & Skills

  • Current unrestricted RN licensure
  • Valid driver's license, vehicle insurance, and reliable transportation or public transit access
  • Preferred: 1+ years clinical experience, CPR certification (or ability to obtain within 90 days)

Compensation & Benefits

The annual salary ranges from $87,560 to $131,340, based on experience and location, with additional benefits such as health coverage, incentive programs, stock purchase options, and retirement plans.
