About the role

As a Registered Nurse Case Manager, you will support patients and families throughout their care journey by coordinating services, developing individualized care plans, and ensuring safe transitions. You will collaborate with physicians, nurses, and the interdisciplinary team to promote quality care, clear communication, and patient-centered outcomes.

Your responsibilities include guiding patients through program orientation, educating families, completing psychosocial assessments, and documenting findings.

Requirements

• Active RN license
• Diploma in nursing
• Minimum of 5 years of experience as a Registered Nurse
• No travel required
