About the Role
This position involves providing patient care, conducting assessments, and managing chronic conditions as a Registered Nurse Case Manager.
Responsibilities
- Evaluate patient conditions and perform assessments within 72 hours of discharge
- Facilitate effective communication during care transitions
- Encourage advanced care planning
- Document patient information in electronic records
- Review labs and diagnostics
- Collaborate with the healthcare team
- Notify families of changes in patient status
- Assist in emergencies
- Provide education to patients and caregivers
Requirements
- High school diploma
- Current RN license
- National RN certification
- Experience in adult, geriatric, home health, or hospice (preferred)
