Registered Nurse Case Manager

About the role

You will support patients and families throughout their care journey by coordinating services, developing personalized care plans, and ensuring safe transitions between care settings.

Responsibilities

• Conduct comprehensive assessments
• Collaborate with interdisciplinary teams
• Manage discharge planning
• Evaluate care progression using evidence-based tools
• Act as a liaison among physicians, patients, families, and external agencies to facilitate effective communication and optimal outcomes

Qualifications and Skills

• Require 2+ years of experience in case management or 3+ years in a clinical setting
• Associates Degree in Nursing (preferably a Master’s)
• Current RN licensure
• BLS certification
• PREFERRED: Experience with InterQual and a strong understanding of care coordination, communication, and documentation
