Location: North/East Ohio and surrounding areas, including New Albany, Gahanna, and Westerville.
About the Role
As a Hospice Registered Nurse Case Manager (RNCM), you will:
- Conduct comprehensive patient assessments, including vital signs and medical history review.
- Travel to patients' residences to conduct visits.
- Develop individualized care plans in collaboration with multidisciplinary teams.
- Coordinate hospice care by working with other team members and the patient's family to ensure needs are met.
- Perform evaluations for hospice care along with informational visits as needed.
- Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care.
- Educate patients and families on treatment plans, medication management, and health maintenance strategies.
- Submit complete, accurate, and relevant clinical notes promptly.
- Participate in IDG and case conferences.
- Perform on-call duties per policy and as needed.
- Conduct an initial and ongoing comprehensive assessment of patients.
- Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care.
- Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed.
- Provide care plan to Hospice Aide and LPN as part of the IDG team.
- All other duties as assigned.
Requirements
- Licensed Registered Nurse.
- BLS Certified.
- Comfortable working in both private homes and facility settings.
- Valid Driver's License and Auto Insurance Coverage.
- Reliable transportation.
- A minimum of 1 year of clinical experience.
- Hospice experience preferred.
Pay
$80,000–$89,000 per year.
Schedule
Monday–Friday, 8:00 AM–5:00 PM.
