Registered Nurse Case Manager

Salary: $80-89k/yr BOE
Schedule: Monday–Friday, 8:00 AM–5:00 PM + on-call rotation
Location: Blue Ash, Ohio and surrounding areas

Requirements

• Licensed Registered Nurse
• BLS Certified
• Comfortable working in both private homes and facility settings
• Valid Driver’s License and Auto Insurance Coverage
• Reliable transportation
• A minimum of 1 year of clinical experience
• Hospice experience preferred

Responsibilities

• Conduct comprehensive patient assessments, including vital signs and medical history review
• Travel to patients’ residences to conduct visits
• Develop individualized care plans in collaboration with multidisciplinary teams
• Coordinate hospice care by working with other team members and the patient’s family to ensure needs are met
• Perform evaluations for hospice care along with informational visits as needed
• Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care
• Educate patients and families on treatment plans, medication management, and health maintenance strategies
• Submit complete, accurate, and relevant clinical notes promptly
• Participate in IDG and case conferences
• Perform on-call duties per policy and as needed
• Conduct an initial and ongoing comprehensive assessment of patients
• Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care
• Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed
• Provide care plan to Hospice Aide and LPN as part of the IDG team
• All other duties as assigned
