Registered Nurse Case Manager

Role: RN Case Manager Responsibilities Coordinate patient care across multiple locations, ensuring safe and efficient transitions through the continuum of care.Conduct comprehensive assessments and develop individualized care plans.Collaborate with physicians, therapists, social workers, and community partners to optimize patient outcomes and reduce readmissions.Provide patient and family education, resource referrals, and perform utilization reviews.Maintain thorough documentation in the EMR system.Engage in ongoing learning, contribute to quality improvement initiatives, and support compassionate, patient-centered care within a collaborative culture.
