Registered Nurse Case Manager

Responsibilities

• Provide direct participant care including acute triage, blood draws, and wound care.
• Assist the care team for provision of participant care including after-hours/weekend on-call rotation.
• Participate in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care and update changes as necessary.
• Attend Interdisciplinary Team (IDT) care plan meetings and Participant/Family meetings as applicable.
• Perform assessments in a variety of settings including PACE Clinic and/or Adult Day Center.
• Develop and provide education to participants, caregivers, and associates as necessary including end-of-life discussions.
• Travel to participant's place of residence, long-term care, and assisted living facilities as required.

Requirements

• Registered Nurse licensure obtained prior to hire date or job transfer date.
• BLS Provider credential obtained within 1 month (30 days) of hire date or job transfer date (American Heart Association or American Red Cross accepted).
• Diploma from an accredited school/college of nursing or required professional licensure at time of hire.

Pay

$35.46–$50.05 per hour
