Registered Nurse Case Manager
About the role
Coordinate the comprehensive interdisciplinary plan of care from admission to discharge, integrating clinical pathways with the unique preferences, cultural values, and goals of the patient and their family.
Continuously monitor patient progress through direct assessments and medical record reviews to evaluate the effectiveness of the health plan and ensure the appropriateness of services relative to the diagnosis.
Proactively identify and eliminate social, financial, or administrative barriers to recovery by advocating for necessary medical resources and facilitating access to post-discharge care options.
Maintain active dialogue with healthcare providers and supervisors to ensure effective plan implementation while serving as a resource for high-quality medical record documentation and regulatory compliance.

Requirements
- Licensed Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required. Licensed as a Registered Nurse in Florida required. Credentialed from State Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.
- BLS Provider preferred. American Heart Association or American Red Cross accepted.
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
