Hospice Registered Nurse Case Manager
Responsibilities
- Conduct comprehensive patient assessments, including vital signs and medical history review
- Travel to patients' residences to conduct visits
- Develop individualized care plans in collaboration with multidisciplinary teams
- Care for patients and families by educating them on treatment plans, medication management, and health maintenance strategies
- Submit complete, accurate, and relevant clinical notes promptly
- Participate in IDG and case conferences
- Perform on-call duties per policy and as needed
- Conduct an initial and ongoing comprehensive assessment of the patients
- Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care
- Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed
- Provide care plan to Hospice Aide and LPN as part of the IDG team
Requirements
- Licensed Registered Nurse
- BLS Certified
- Comfortable working in both private homes and facility settings
- Valid Drivers License and Auto insurance Coverage
- Reliable transportation
- A minimum of 1 year of Clinical experience
- Hospice experience preferred
