Schedule: Day Shift | Full-time
Responsibilities
- Orchestrate the full interdisciplinary plan of care from admission through discharge to ensure a seamless patient experience.
- Design personalized care strategies that integrate clinical requirements with patient and family values and preferences.
- Monitor patient progress and oversee medical documentation to ensure treatment options and services align with diagnostic needs.
- Advocate for essential resources and proactively remove barriers to ensure the effective delivery of the health plan.
- Maintain continuous collaboration with supervisors and providers to optimize care implementation and post-discharge success.
Requirements
- Licensure / Certification / Registration: Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
- Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
- Education Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
