Registered Nurse - Case Management

Responsibilities

• Responsible for the quality and resource management of all patients admitted to the facility from the point of admission and across the continuum of health care management.
• Works on behalf of the advocate, promoting cost containment and demonstrating leadership to integrate health care providers for seamless care delivery.
• Performs holistic and comprehensive admission and concurrent reviews of medical records for medical necessity, intensity of service, and severity of illness.

Requirements

• Minimum 5 years of work experience post-graduation from an accredited school of nursing and a current state Registered Nurse license (starting April 1, 2015). Grandfathered prior to April 1, 2015.
• For Licensed Clinical Social Worker: Minimum 5 years post-graduate experience from an accredited school of Social Work.
• RN Case Manager preferred.
• Five years of acute care nursing experience preferred.
• At least one year of experience in case management, discharge planning, or nursing management preferred.
• Current BCLS (AHA) certificate upon hire and maintained thereafter.
• Knowledge of Milliman Criteria and InterQual Criteria preferred.
• Experience and knowledge in basic to intermediate computer skills.
• Behavioral Violence Prevention Training within 3 months of hire and maintained thereafter.
