Summary of Position

Provide professional nursing care to patients and proactively support efforts to ensure that standards for patient satisfaction, patient safety, and quality of care are maintained, as established by AdvantageCare Physicians. Serve as the primary liaison between patients, staff, and physicians, and support the Advanced Care Model. Organize and provide clinical supervision to the specialty patient care team, including delegation of responsibilities, mentoring, evaluating, educating, scheduling, and counseling the care team staff.

Responsibilities

• Provide general nursing care to patients, including administering prescribed medications and treatments in accordance with nursing standards and physician orders.
• Plan, articulate, and coordinate care and interventions, considering age-related characteristics, physical development, communication, socialization, and safety, in collaboration with the care team.
• Ensure nursing practices of the specialty care team and own practices comply with nursing standards, local, state, and federal guidelines, and organizational policies.
• Provide clinical supervision to Licensed Practical Nurses (LPNs) and Care Team Associate IIs (CTA IIs), assist with scheduling, delegate tasks, and follow up on delegated tasks.
• Function as the Lead Nurse in the medical office when the Manager, Nursing (and/or Assistant Manager, Nursing) is unavailable.
• Assist with new hire orientation, staff training, performance evaluations, and employee discipline/termination processes.
• Coordinate and manage daily huddles, prepare materials, and review patient charts for missing labs, reports, and open orders.
• Manage clinical supplies in coordination with the Manager, Nursing, and Practice Administrator.
• Prepare equipment and assist physicians during treatments, examinations, and procedures.
• Verify chief complaints, take vital signs, review EHR medication lists, conduct point-of-care tests, and administer medications as directed.
• Contact patients with test results, provide recommendations, and update individualized care plans.
• Facilitate clinical messages, chronic condition management education, and document progress notes in the EHR within 24 hours.
• Manage in-basket requests, triage urgent appointments, and coordinate outpatient work-ups and treatment plans.
• Pend tests and procedure orders, prepare prescriptions, and provide patient education on self-management of chronic conditions.
• Administer, treat, and train patients on self-administered drugs.
• Participate in nursing practice work groups and committees as required.

Qualifications

• Education: Graduate of an accredited school of nursing; Bachelor’s Degree in Nursing preferred.
• Licenses/Certifications: Current unencumbered New York State Registered Nurse license; BLS; ACLS required for GI and Cardiology.
• Experience: Work experience as a nurse in a hospital or ambulatory setting preferred; experience in specific specialty (e.g., Cardiology, Gastroenterology) preferred; experience with AAAASF Accreditation (GI position only) preferred.
• Skills: Strong knowledge of professional nursing theory, regulations, and practices; ability to maintain quality control standards; excellent verbal and non-verbal communication skills; knowledge of electronic medical records (EMR) preferred.

Pay

Hiring Range: $81,000-$96,900
