Responsibilities

• Ensure safe and appropriate nursing services and medication administration for persons supported across assigned caseload, collaborating with regional, nursing, and other leadership and interdisciplinary teams to implement person-centered, individualized plans of nursing service.
• Serve as the primary resource for day-to-day nursing and medical-related needs of the people supported, providing leadership and clinical support to program staff, attending regional leadership meetings, and implementing resolutions to address new or changing needs.
• Develop and maintain an understanding of regulations applicable to assigned programs/systems as they relate to medical and nursing care, including specific medical conditions of persons supported and scopes/standards of nursing practice in the state(s) of assignment.
• Directly supervise Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) for assigned caseload, ensuring medical needs of persons supported are met.
• Maintain regular communication with assigned nursing staff to ensure consistent application of job expectations, performance standards, policies, and procedures; conduct regular observations to evaluate performance and manage concerns through corrective actions or development plans.
• Ensure nursing staff utilize the full nursing process for each person supported, from assessing conditions to improving care effectiveness, and communicate appropriately based on individual learning needs related to healthcare.
• Regularly monitor and assess nursing caseloads, proposing changes or new needs to the nursing leadership team, including reassignments to address specific needs or concerns.
• Direct and assist assigned nursing staff with implementing, monitoring, and updating plans of nursing care (PONS), addressing new and changing medical needs, and handling complex medical issues through support, feedback, and coaching.
• Travel regularly to programs across the assigned region to engage with or observe persons supported, program staff, and nursing staff; conduct assessments, implement plans, collect data, facilitate medical in-services, and attend meetings.
• Participate in the hiring process, interviewing and assessing candidates for nursing roles, and ensure appropriate staffing and coverage for assigned caseload.
• Initiate, review, or approve staff status changes (e.g., work hours, promotions, transfers) and ensure timely submission in Workday.
• Ensure accurate and timely completion of timekeeping records and approvals in Workday to support payroll processing; address attendance or time-tracking concerns.
• Collaborate with regional program leadership, program staff, and the nursing team to identify and address clinical training needs, supports, assessments, and related plans.
• Plan and facilitate regional direct care leadership and clinical team meetings, providing support, guidance, mentorship, and feedback to Health Care Specialists and other direct care staff; directly supervise Regional Health Care Specialist(s).
• Regularly monitor medical systems, including Electronic Medication Administration Record (eMAR), to ensure quality documentation and adherence to prescribed orders for medication administration.
• Support nursing leadership with implementation and communication of new or updated regulations, protocols, and infection control practices; provide guidance and education to assigned caseload.
• Review medical data for assigned caseload through regular monitoring of electronic health records and eMAR to support quality assurance; analyze documentation to identify trends (e.g., medication errors, 911 calls, falls).
• Collaborate with applicable parties to address audit findings and implement corrective action plans, facilitating trainings to address deficiencies and improve health outcomes.
• Participate in the nursing call list for assigned caseload, triaging phone calls, including escalated calls from nursing staff, and handling inquiries from program staff, leadership, or persons supported during and outside regular working hours.
• Present information and respond to inquiries regarding the health, conditions, and status of persons supported; assist with nursing-related needs in other service settings as needed.
• Ensure appropriate oversight of medical care and systems by nursing staff, including regular in-service training for direct care staff on medical needs, medication administration, and specific conditions.
• Lead intake assessments during the referral process, reviewing medical records, suggesting program matches, and facilitating direct observations to inform placement recommendations.
• Participate in the residential nursing on-call system on a rotational basis to ensure 24/7 coverage and response to incidents, inquiries, or emergencies within 30 minutes, in accordance with OPWDD guidelines.
• Stay abreast of applicable federal, state, and local regulations and standards, including those related to oversight agencies, infection control, and nursing care; complete and maintain all necessary trainings.
• Provide support to the agency through participation in program and nursing-focused initiatives, committees (e.g., Human Rights, Incident Review, Choking Committees), and special projects as requested.
• Participate in the development of medical in-service trainings or assessments for specific content areas (e.g., AMAP, choking, diabetes) and specialized programs (e.g., START, OMH).
• Ensure compliance with all federal, state, local, and regulatory agency requirements, including the Health Insurance Portability and Accountability Act (HIPAA).
• Perform all other duties as assigned.

Requirements

• Valid, active Registered Nursing (RN) license and/or registration to practice as an RN in the assigned state of operations (NY or NJ).
• Current Basic Life Support (BLS) certification for Healthcare Providers.
• Bachelor of Science in Nursing (BSN) degree and three (3) years of nursing experience in any healthcare setting, preferably working with individuals with intellectual and developmental disabilities (I/DD); or a satisfactory equivalent combination of education, experience, and/or training. All incumbents must hold a current, valid RN license in the state of assignment and current BLS certification.

• Exceptional supervisory and leadership skills, including the ability to direct, coach, and support nursing and program staff across multiple sites through hands-on support and adaptive leadership.
• Ability and willingness to lead, direct, and teach program staff on appropriate medical support for persons supported, including medication administration and related care.
