Registered Nurse Care Manager

About the role

This Care Manager team member will be responsible for:

• Making home visits to meet potential and current clients.
• Assessing clients' needs and establishing a care plan.
• Coordinating and monitoring care provided by caregivers.
• Overseeing the training and performance of new caregivers.
• Maintaining compliance with policies and regulations.

Responsibilities

This Care Manager team member will be responsible for:

• Meeting potential and current clients in their homes to listen to their stories, determine their needs, and establish a plan of care during an initial assessment.
• Managing the coordination of care and maintaining compliance with policies and procedures and all state home care rules and regulations.
• Providing home visits on an as needed basis or every 90 days to monitor any changes in client condition.
• Overseeing the Caregivers providing care to our wonderful clients.
• Aiding in New Hire Orientation and Training of new Caregivers, as well as performing skills assessment on New Hires.
• Observing Caregivers in the home and assisting in employee reviews.

Requirements

Minimum qualifications include:

• Two years of Registered Nurse experience preferred but not required in home care, home health or related field.
• Reliable transportation with current insurance and a valid driver’s license.
• Excellent written and verbal communication skills and ability to interact effectively with a diverse audience including clients, families, care providers and other professionals.
• Impeccable follow up and attention to detail, and ability to exercise initiative, problem solve, apply sound judgment.
• Able to supervise, assess and evaluate care providers and advocate on behalf of clients and families.
• Passionate about helping others live with dignity.
• Able to present a positive and professional business image.
• Passes a criminal background check.

Qualifications

Qualifications include:

• North Carolina Registered Nurse license.
• Familiarity with home care and/or related fields.

• Strong organizational and time management skills.
• Ability to work independently and as part of a team.
