Registered Nurse Care Manager

Role: RN Health Plan Care Manager Responsibilities Coordinate whole-person care for health plan members through telephonic and virtual assessments.Develop individualized care plans in collaboration with providers, social services, and caregivers.Manage chronic conditions, support care transitions, and promote prevention and wellness.Reduce hospitalizations and improve health outcomes while ensuring the appropriate and cost-effective use of services.Participate in a team dedicated to serving communities in a mission-driven, faith-based environment. Qualifications & Skills RN licensure with relevant experience in care management or similar roles.Strong communication and collaboration skills for virtual assessments and care planning.Knowledge of chronic condition management, care transitions, and preventive care.Commitment to compassionate, patient-centered care and professional growth.
