Registered Nurse Care Manager - Advocate Ambulatory Care Coordination and Management

Responsibilities
- Provide comprehensive care in ambulatory settings, including but not limited to Primary Care, Urgent Care, and specialty clinics.
- Collaborate with a multidisciplinary team to ensure high-quality patient care.
- Participate in professional governance and contribute to evidence-based practice initiatives.
- Shape the future of health through innovation, research, and education.
Requirements
- Must reside in North Carolina.
- Completion of an accredited or approved program in nursing.
- Registered Nurse license issued by the state of North Carolina.
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association.
Qualifications
- BSN preferred.
- Minimum 2 years of RN experience preferred.
- Prior Ambulatory, Family Medicine, and NC Medicaid experience preferred.
- Care Management certification preferred.
Schedule
Monday to Friday, 8:00 AM to 5:00 PM. Remote work available.
