About the role
We’re seeking a dedicated Registered Nurse (RN) to play a pivotal role in our Palliative Care team. As a RN with us, you'll be instrumental in delivering top-notch, evidence-based care to a panel of seriously ill patients. Responsibilities
- Patient Care: Deliver nursing services aligned with individualized care plans under the direction of the patient's provider, assessing treatment effectiveness and the patient's physical and emotional responses.
- Collaborate in interdisciplinary planning and case conferences, ensuring care plans are up to date, goals are relevant and feasible for the care team's capabilities, and concerns are escalated appropriately.
- Record and communicate patient observations accurately within HIPAA-compliant systems and medical records.
- Facilitate case management and care coordination to uphold patient care plans effectively.
- Manage assigned patients, ensuring appropriate visit frequencies and addressing evolving acuity levels and clinical needs.
- Educate patients and caregivers about VyncaCare services, palliative care, disease processes, symptom management, and medication management.
- Assist with the in-person completion and documentation of advanced care planning discussions, including POLST forms and related care preference documentation.
- Float Nurse: Assist in covering patient care for other care teams during normal working hours.
- Coordinate administrative tasks, including strong collaboration with scheduling team to identify travel dates, coordination of all travel arrangements including hotel, flights, etc., and occasionally overtime might be required for travel.
Requirements
- Active, unrestricted California Registered Nurse (RN) license.
- 2+ years of clinical nursing experience in home health or community health.
- Direct experience in palliative care or hospice is highly preferred.
- At least 1 year of professional nursing experience within the past three years.
- Demonstrated proficiency in nursing practices, clinical judgment, and documentation.
- Strong oral and written communication skills.
- Strong decision-making skills and ability to work independently.
- Valid driver's license, insurance, and reliable vehicle.
- Proficiency in EMR, Google Suite, and other relevant technologies.
- Willing and able to work Monday through Friday 8:30am to 5pm Pacific Time, with flexibility for potential evenings and weekends.

Pay
The pay range for this role is $60,000 - $70,000 annually.
Schedule
This position requires consistent travel throughout the below territory, with primary travel occurring throughout Humboldt, Coastal Redding, Shasta, and Far North areas. Candidates wishing to be considered must reside within 25-miles of the assigned territory due to frequency of travel.
