About the Role
TwelveStone Infusion Centers manages the infusion needs of patients and maintains the continuum of care, from intake, assessment, and treatment through the completion of their visit. Our primary focus is the safe and effective administration of biologic medications (e.g., Remicade, Tysabri, etc.), but we also offer a full range of infusion services including antibiotics, hydration, vitamins, iron, and steroids. On occasion, there is a need for sterile dressing changes and lab draws.
Responsibilities
- Perform analysis of patient data including status of lab work and patient assessments.
- Report changes to referring physician and make recommendations for patient care.
- Perform initial and ongoing patient assessments.
- Provide patient education; participate in clinic quality improvement process.
- Conduct individualized patient assessments, prioritizing data collection based on the patient’s immediate condition or needs within specified timeframes.
- Administer medications, infusion therapy, and other appropriate treatments per prescriber orders under the supervision of the clinic mid-level provider.
- Document, verify, and ensure implementation of physician orders, accurate and timely clinical documentation.
- Document patient assessment findings, physical/psychosocial responses to nursing intervention, and progress toward problem resolution.
- Deliver care with appropriate age and cultural competence to specific patient populations according to individualized needs.
- Maintain confidentiality in matters related to patient, family, and client facility staff.
- Provide care in a non-judgmental, non-discriminatory manner that is sensitive.
Requirements
- Bachelor’s degree in nursing or Associate degree in Nursing.
- Current, unrestricted RN license in the state of practice (Virginia).
- Current CPR certification (BLS minimum, including hands-on/in-person assessment).
- Minimum of one year’s experience providing nursing care preferred.
- Infusion experience preferred.
- Telephone, MS Office, and computer/keyboard skills required.
- Previous EMR (Electronic Medical Records) experience preferred.
