About the role
Ozanam Manor (I and II) Interim Housing Communities are transitional housing programs for 160 single adults experiencing homelessness who are 50+ or disabled, including Veterans. Nursing care is necessary for homeless Veterans who have some type of unstable or uncontrolled medical issue that requires a level of oversight.
Responsibilities
- Medical Recuperative Care Contract Resident Veterans (No less than 20 hours per week for up to 5 Residents)
- Providing weekly education and assistance with medication management and treatment compliance within the scope of practice for nursing.
- Maintaining a timely and thorough electronic clinical record that documents medical prescriptions and orders issued by physicians and that documents nursing notes in a professional manner.
- Supporting coordination of care with VA providers and the VA Liaison to ensure continuity of care.
- Overseeing and assisting with coordination of Veteran participation in in-home health care and outpatient services to address varying recuperative needs (i.e. Physical Therapy, Occupational Therapy, Wound Care, etc.).
- Participating in weekly case conferencing to monitor resident progress, engage in team problem solving, and provide education to case managers on resident medical concerns.
- Addressing minor medical concerns of Residents with appropriate over the counter (OTC) remedies.
- Maintaining timely and thorough documentation in established electronic system.
- Acting as healthcare resource for Ozanam staff.
- Administering first aid to Residents as needed.
Healthcare Oversight
- Identifying Residents with higher healthcare needs and lesser ability to manage their healthcare and activities of daily living (ADL).
- Assisting residents with navigating the healthcare system, obtaining appropriate medical care, and making appointments as needed.
- Ensuring transportation is arranged for medical appointments.
- Assisting residents with the use of telehealth as needed.
- On a case-by-case basis, accompanying residents to medical appointments.
- Participating in weekly case conferencing to monitor resident progress, engage in team problem solving, and provide education to case managers on resident medical concerns.
- Maintaining timely and thorough documentation in established electronic system.
- Overseeing the medication management processes of the Program, including arranging for Residents to take medication assisted therapy (MAT) at their clinic, physician’s office, or another off-site location.
- When consistent with the individual’s treatment plan, storing a Resident’s MAT medication in a lock box within the Program for the individual to be personally responsible for self-administration.
Requirements
- Licensed as a Registered Nurse in the state of Arizona.
- Valid driver’s license with a clean driving record.
Preferred Skills/Abilities
- Excellent communication, organization skills, and ability to interact with guests, staff, and volunteers required.
- Related experience working with vulnerable populations in a transitional housing or shelter environment preferred.
- Knowledge of available community support and services is beneficial and will be shown preference.
- Strong computer skill, proficiency with Microsoft Suite programs, and excellent electronic communication and documentation preferred.
