Registered Nurse
Position Overview
The Registered Nurse will work on OMH care teams with mental health professionals assisting clients in transitioning from hospitals back to the community. The team follows individuals into the community using the Evidence-Based Critical Time Intervention model of care to ensure they are linked and engaged with identified community providers and have sufficient support networks to meet their needs.
The Registered Nurse provides diagnosis and treatment of human responses to actual or potential health problems through services like medical case management, referrals, health teaching, and counseling. They also provide interventions supportive to or restorative of life and well-being, executing medical regimens prescribed by healthcare providers.
The role requires limited on-call coverage. The Patchen Residence Program is a congregate Level II Co-Occurring Mental Health and Substance Misuse transitional facility for males under the Office of Mental Health.
Essential Duties & Responsibilities
- Complete medical assessments and communicable disease risk assessments within mandated time frames.
- Cook for self-administration of medications for detoxification according to established protocols.
- Refer for required medical services.
- Provide education and consultation to individual clients and/or groups on their health needs.
- Assess clients' medical records and coordinate medical care with multidisciplinary team.
- Assure clients' compliance with prescribed medical regimen.
- Complete timely and accurate documentation on medical services rendered.
- Keep supervisor informed of all relevant information regarding individuals and the program.
- Provide internal training to individuals/staff members.
- Participate in weekly protocol meetings.
- Perform other duties as requested by supervisor.
Qualifications
- NYS RN licensure
- Knowledge of primary diagnosis
- Excellent written and verbal communication skills
- PREFERRED QUALIFICATIONS & SKILLS
- Assessment and case management
- Basic computer skills (e.g., MS Office suite and Windows)
- Ability to provide crisis intervention utilizing good interpersonal skills with a high level of maturity and responsibility
