About the role
As a Registered Nurse (RN) or Outpatient Nurse, you will:
- Conduct in-home or community-based assessments
- Perform head-to-toe evaluations, including medication and diagnosis review
- Complete aide supervisions and provide clinical guidance
- Develop and update personalized Plans of Care (POC)
- Provide orientation to aides based on the POC
- Accurately document findings and maintain compliance with healthcare regulations
Requirements
- Current New York State RN license
- Ability to travel throughout Rockland County
- Medical Professional Liability insurance
- Comfortable using Gmail, Microsoft Word/Excel
Preferred Qualifications
- Bilingual in Spanish/English
- Familiarity with Medicaid, Medicare, or MLTC plans
- Experience in outpatient nursing, home care, or community health
- Strong documentation and communication skills
