About the Role

RiverStone Health Hospice Services provides comfort-oriented care for terminally ill patients, emphasizing quality of life. Care and grief support are delivered wherever the patient calls home, including private residences, nursing homes, or other facilities. Our interdisciplinary team of professionals and volunteers works together to relieve physical symptoms and address psychological, social, emotional, or spiritual distress for patients and their caregivers. 24-hour on-call nursing services are available to meet urgent needs.

This is a part-time, non-exempt position reporting to the Hospice Program Manager.

Responsibilities

  • Provide direct end-of-life care and instruction for patients and families based on established clinical nursing practice standards.
  • Direct symptom management and medication administration in compliance with physician’s orders and plan of care; communicate changes in patient condition to the primary medical provider and coordinate treatment modifications.
  • Monitor and interpret symptoms and treatments; develop and implement care plans in collaboration with an interdisciplinary team.
  • Provide education and support to patients and families; assess physical, social, and environmental needs to develop a treatment plan.
  • Evaluate patient referrals, conduct admissions, and perform nursing visits as assigned.
  • Initiate and prioritize the plan of care to ensure effective and efficient patient care delivery.
  • Consult and coordinate with interdisciplinary team members to assess, plan, implement, and evaluate patient care plans.
  • Follow a plan of care to identify, prioritize, and meet patient and family needs to achieve clinical outcomes.
  • Ensure care, educational, and end-of-life planning needs of patients are met.
  • Direct and supervise Licensed Practical Nurses (LPN) and Certified Home Health Aides (CHHA), including supervisory visits.
  • Communicate patient condition changes to physicians and families as needed; provide reports to team members for continuity of care.
  • Demonstrate knowledge of growth and development principles across the lifespan to provide age-appropriate care.
  • Document patient encounters in electronic medical records per documentation standards, including observations, activities, and clinical condition.
  • Provide hygiene and personal care to patients.
  • Maintain compliance with Medicare/Medicaid, state regulations, HIPAA, and hospice accreditation standards.
  • Participate in program quality improvement projects.
  • Keep supervisor informed of work progress, challenges, and suggestions for improvement.
  • Attend meetings, conferences, workshops, and training to stay current on best practices.
  • Rotate on-call duties to ensure patient care availability after hours, weekends, and holidays, including triage, admissions, symptom management visits, education, and attendance at patient deaths.
  • Perform other duties as assigned to support RiverStone Health’s mission.

Requirements

  • Associate degree in Nursing (Bachelor of Science in Nursing preferred).
  • Current Montana Registered Nurse license.
  • Valid Montana Driver’s License and vehicle insurance.
  • Current BLS certification.
  • Computer literacy in Microsoft Office Suite.
  • Ability to maintain a calm and positive demeanor during difficult interactions.
  • Non-judgmental and empathetic listening skills.
  • High attention to detail.
  • Knowledge of HIPAA and confidentiality requirements.
  • Ability to work collaboratively in a positive work environment.
  • Ability to prioritize tasks under pressure and continuous interruptions.
  • Strong communication skills for efficient and quality customer service.

Preferred Qualifications

  • Experience with hospice or palliative care.
  • Knowledge of nursing processes and hospice/palliative care regulations.

Schedule

Part-time, Thursday and Friday, 8:00 AM to 5:00 PM, with rotating on-call duties after hours, weekends, and holidays.

Pay

Wage range: $34.52 to $50.04 per hour, depending on years of transferrable experience and internal equity.

Working Conditions

  • Physical demands include standing, walking, sitting, talking, hearing, and using hands to handle objects or controls.
  • Regularly required to stoop, kneel, bend, crouch, and lift up to 25 pounds frequently and 50 pounds occasionally; assistance or lifts may be required.
  • Vision requirements: close, distance, depth perception, color vision, and ability to adjust focus.
  • Ability to travel using a personal vehicle in all weather conditions.
  • Exposure to biological pathogens, contagious diseases, and home environments with potential allergens (e.g., smoke, pets).
  • Risk of needlestick injury; adherence to safety protocols and PPE use is required.
