Registered Nurse

Home Health RN Responsibilities

• Provide skilled nursing care in patient homes, including assessments, wound care, medication management, and treatments.
• Develop and implement individualized home health care plans.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and families on disease management, rehab, and senior care.
• Carefully coordinate care with physicians, therapists, and the Director of Nursing.
• Document patient visits in compliance with home health regulations.

Qualifications

• Active California Registered Nurse (RN) license (required).
• Minimum 1 year RN experience (home health experience preferred).
• Experience in rehab, geriatric care, or community health is a plus.
• Strong clinical, communication, and organizational skills.
• Valid driver’s license and reliable transportation.
• Able to travel throughout Los Angeles County – Santa Clarita, Valencia, Canyon Country.
