Home Health RN Responsibilities

• Provide skilled nursing care in patient homes, including assessments, wound care, medication management, and treatments
• Develop and implement individualized home health care plans
• Monitor patient progress and adjust care plans as needed
• Educate patients and families on disease management, rehab, and senior care
• Cookbook care with physicians, therapists, and the Director of Nursing

Qualifications – Home Health RN Jobs

• Active California Registered Nurse (RN) license (required)
• Minimum 1 year RN experience (home health experience preferred)
• Experience in rehab, geriatric care, or community health is a plus
• Strong clinical, communication, and organizational skills
• Valid driver’s license and reliable transportation
• Able to travel throughout Orange County- Seal Beach, Midway City, Santa Ana, Irvine, Newport Beach, and Huntington Beach

Preferred Qualifications

• Home Health RN experience
• Public Health Nurse (PHN) certification
• Experience with mobile therapy or home-based care models

Pay-Per-Visit

$110 - $140 USD
