Position Overview
Entyre Care helps elders and people with disabilities stay healthy at home with trusted caregivers. As an RN Supervisor, you are the clinical backbone of that model— you assess clients in their homes, build their plans of care, and train and mentor their caregivers. This is hands-on, in-the-field nursing with real autonomy. You will report to the Program Director and help shape how a growing agency delivers care. This role serves as the operational bridge between members, family caregivers, managed care organizations, and Entyre’s internal teams to ensure a seamless and compliant care experience.
Requirements
- Active, unrestricted Wisconsin RN license (or multistate compact privilege)
- Experience in home care, personal care, or community-based nursing preferred
- Comfort working independently in clients' homes across Milwaukee County
- Valid driver's license and reliable transportation
- Strong documentation habits—accuracy here protects clients and the agency alike
What You Will Bring
- Assess & Plan: Conduct in-home client assessments and evaluate needs against nursing standards; develop individualized plans of care following each home visit, reflecting client preferences and physician direction; obtain and maintain signed physician orders; renew every three months or on a change in condition; review and update plans every 60 days, or whenever a client's condition changes
- Train & Mentor: Provide written instruction and hands-on training so personal care workers demonstrate skill mastery before working independently; delegate clinical tasks, evaluate competency, and retrain where gaps appear; support new caregivers through orientation and offer ongoing mentorship in the field
- Supervise & Improve: Conduct scheduled and unannounced supervisory home visits at least every 60 days—observing care delivery, auditing daily records, and documenting any changes needed; serve on the Quality Assessment and Assurance Committee and contribute to operational planning; coordinate referrals to additional services as client needs evolve
