Position Overview
Polaris Rehabilitation & Care Center is seeking a dedicated and compassionate Registered Nurse (RN) to join our clinical team. This role is ideal for a nurse who is comfortable providing direct resident care while also serving as a clinical leader and resource for nursing staff during the assigned shift.
Key Responsibilities
- Provide and oversee nursing care in accordance with individualized care plans, physician orders, and established clinical standards
- Perform ongoing assessments of resident conditions and respond appropriately to changes in health status
- Administer medications, treatments, and therapeutic interventions safely and accurately within scope of practice
- Collaborate with physicians, providers, therapy, pharmacy, and interdisciplinary team members to coordinate resident care
- Supervise, delegate, and support CNAs and other nursing staff during assigned shifts
- Provide education and guidance to residents and families regarding care plans, medications, treatments, and health management
- Maintain accurate, timely, and compliant documentation within the electronic medical record system
- Participate in infection prevention, resident safety, quality assurance, and performance improvement initiatives
- Respond to urgent or emergency situations using sound clinical judgment and established protocols
- Ensure compliance with facility policies, professional standards, resident rights, and applicable federal and Wyoming regulations
- Communicate changes in resident condition promptly to physicians, families, and appropriate members of the care team
- Promote resident dignity, comfort, independence, and quality of life
Qualifications
- Graduate of an accredited nursing program
- Current, active Registered Nurse (RN) license in good standing in the State of Wyoming
- Current CPR or Basic Life Support certification
- Previous experience in skilled nursing, rehabilitation, long-term care, or post-acute care preferred
- Strong clinical assessment, critical thinking, and decision-making skills
- Demonstrated ability to supervise, delegate, and support nursing staff effectively
- Excellent communication and interpersonal skills
- Able to manage multiple priorities in a fast-paced healthcare environment
- Strong attention to detail and commitment to accurate clinical documentation
- Commitment to delivering compassionate, high-quality, resident-centered care
