Position Overview

Polaris Rehabilitation & Care Center is seeking a dedicated and compassionate Registered Nurse (RN) to join our clinical team. This role is ideal for a nurse who is comfortable providing direct resident care while also serving as a clinical leader and resource for nursing staff during the assigned shift.

Key Responsibilities

  • Provide and oversee nursing care in accordance with individualized care plans, physician orders, and established clinical standards
  • Perform ongoing assessments of resident conditions and respond appropriately to changes in health status
  • Administer medications, treatments, and therapeutic interventions safely and accurately within scope of practice
  • Collaborate with physicians, providers, therapy, pharmacy, and interdisciplinary team members to coordinate resident care
  • Supervise, delegate, and support CNAs and other nursing staff during assigned shifts
  • Provide education and guidance to residents and families regarding care plans, medications, treatments, and health management
  • Maintain accurate, timely, and compliant documentation within the electronic medical record system
  • Participate in infection prevention, resident safety, quality assurance, and performance improvement initiatives
  • Respond to urgent or emergency situations using sound clinical judgment and established protocols
  • Ensure compliance with facility policies, professional standards, resident rights, and applicable federal and Wyoming regulations
  • Communicate changes in resident condition promptly to physicians, families, and appropriate members of the care team
  • Promote resident dignity, comfort, independence, and quality of life

Qualifications

  • Graduate of an accredited nursing program
  • Current, active Registered Nurse (RN) license in good standing in the State of Wyoming
  • Current CPR or Basic Life Support certification
  • Previous experience in skilled nursing, rehabilitation, long-term care, or post-acute care preferred
  • Strong clinical assessment, critical thinking, and decision-making skills
  • Demonstrated ability to supervise, delegate, and support nursing staff effectively
  • Excellent communication and interpersonal skills
  • Able to manage multiple priorities in a fast-paced healthcare environment
  • Strong attention to detail and commitment to accurate clinical documentation
  • Commitment to delivering compassionate, high-quality, resident-centered care
