About the role
The Registered Nurse (RN) at Oak Park Place is responsible for providing thoughtful care to residents based on their personalized Plan of Care. This includes monitoring and tracking changes in residents' conditions and needs, and proactively updating relevant contacts about these changes and concerns.
Responsibilities
- Maintain thorough documentation of residents' care plans and progress
- Monitor residents' health status and report significant changes to healthcare providers
- Communicate effectively with residents, families, and healthcare teams to ensure coordinated care
- Adapt care plans as needed based on residents' evolving needs
- Ensure compliance with all regulatory standards and guidelines
Requirements
- Current LPN license (#OPP)
- Previous experience in nursing or related medical field
- Familiarity with medical software and equipment
- Strong interpersonal and communication skills
- Ability to solve problems and think critically
- Willingness to work in a fast-paced environment
Qualifications
- High school diploma or equivalent
- Bachelor's degree in Nursing preferred
- Current CPR certification
Skills

- Ability to work independently and as part of a team
- Knowledge of long-term care principles and practices
