Responsibilities
- Conduct comprehensive nursing assessments in patients’ homes
- Deliver skilled nursing care including wound care, medication management, treatments, and patient monitoring
- Develop, implement, and update individualized home health care plans
- Educate patients and caregivers on disease management, medications, safety, and ongoing care needs
- Collaborate with physicians, case managers, therapists, and interdisciplinary care teams
- Complete accurate and timely clinical documentation in compliance with agency and regulatory standards
- Monitor patient progress and communicate changes in condition to the care team
- Support improved patient outcomes, independence, and quality of life
Requirements
- Current Arizona Registered Nurse (RN) license in good standing
- Graduate of an accredited nursing program (BSN preferred)
- Minimum of 1 year of RN experience required
- Home Health nursing experience strongly preferred
- Experience working with geriatric patient populations preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and organizational abilities
- Valid driver’s license, reliable transportation, and ability to travel throughout the East Valley areas
- Ability to work independently while delivering compassionate mobile care
