We are looking for a team member who is energetic, very well organized, able to multi-task, has a warm telephone manner and excellent customer service skills. Must represent the agency with the highest professionalism. A strong work ethic and the ability to interact well with a wide variety of people.

QUALIFICATIONS

• Experience with seniors a plus but not required

RESPONSIBILITIES

• Perform non-medical comprehensive assessments
• Develop an individualized plan of care for each client with client input
• Oversee of non-medical client care
• Conduct client reassessments and follow up visits
• Provide training, delegation, supervision, performance oversight and support to our staff of CHHAs
