About the role
The Center for Human Development (CHD) is seeking a Registered Nurse (RN) for our Adult Community Clinical Services (ACCS) in Holyoke, MA. ACCS, funded through the Department of Mental Health, is a multidisciplinary outreach program for individuals needing psychiatric and other services while living independently in the community. This role involves active engagement, including assertive outreach, consistent assessment and treatment planning, skill building, symptom management, addiction treatment, and behavioral and physical health monitoring and support.
ACCS teams include licensed clinicians, RNs, housing specialists, substance use specialists, direct care staff, and peer/family specialists. The team collaborates to provide person-centered, evidence-based services supporting individuals' treatment goals. The RN coordinates health-related functions, provides direct support to assess needs, participates in medical reviews, offers counseling on health and psycho-physiological well-being, and liaises with medical and community-based providers. This is an administrative RN role.
Responsibilities
- Coordinate health-related functions and activities to promote, maintain, and enhance the well-being of individuals served.
- Provide direct support to assess individual health needs.
- Participate in medical reviews and provide health and psycho-physiological counseling.
- Liaise with medical and community-based providers.
- Work collaboratively with a multidisciplinary team to deliver person-centered, evidence-based services.
Requirements
- Massachusetts Registered Nurse License required.
- Bachelor’s degree preferred.
- Two years of experience working within mental health and/or substance use field preferred.
- MAP (Medication Administration Program) experience preferred.
- Residential experience preferred.
- Acceptable driving record.
