About the role
As a hospice Registered Nurse Case Manager, you will perform ongoing patient assessments, maintaining thorough and diligent notes and records regarding the effectiveness of each patient's care plan. You will also keep in constant contact with patient family members and alternative caregivers.
Responsibilities
- Providing nursing care with an emphasis on pain and symptom management
- Supervising Home Health Aides
- Referring patients' families to applicable community resources
- Maintaining strict patient confidentiality
- Providing patient care directives to family members and alternative caregivers
Requirements
- Licensed as a Registered Nurse in the state of Pennsylvania
- Associate, Bachelor's, or Master's Degree in nursing
- Minimum of 2 years Med-Surg, Home Care, and/or Hospice experience
- Understand the difference between Progressively Declined and Custodial Care
- Ability to occasionally lift up to 25 lbs
- Absolute integrity and sound judgment
- Act as a patient advocate, working independently to ensure patients are physically comfortable and content
- Good social skills, energetic, organized, and emotionally capable of caring for terminally ill individuals
- Team player, able to work harmoniously with a wide variety of individuals
Bilingual (Spanish, Mandarin) is a plus.

Schedule
8:30 am – 5:00 pm
Must be available to work occasional weekends.
