Registered Nurse

About the role

We are seeking a compassionate and experienced Home Health Care Registered Nurse (RN) to join our dynamic team. The ideal candidate will have expertise in utilizing the OASIS system, maintaining meticulous patient records, and delivering skilled nursing care in patients' homes.

Responsibilities

• Conduct thorough initial and ongoing patient assessments using the OASIS tool to develop individualized care plans.
• Provide skilled nursing care, including wound care, infusion therapy, catheter management, and other specialized services as prescribed.
• Educate patients and families on managing health conditions, medications, and preventative care.
• Maintain accurate, detailed, and timely documentation of patient conditions, interventions, and progress in compliance with federal, state, and agency guidelines.
• Submit OASIS documentation promptly to ensure compliance with Medicare and agency standards.
• Communicate changes in patient status to the care team and implement appropriate interventions.
• Serve as a liaison between the patient, their family, and the care team to address questions and concerns.
• Adhere to best practices and evidence-based protocols to ensure the highest standard of care.

Requirements

• Valid nursing license
• Minimum of 1 year of home health care nursing experience
• Experience administering medication via PEG Tube (percutaneous endoscopic gastrostomy)
• Proficiency in OASIS documentation
• Experience in wound care and infusion therapy
• Knowledge of Kinser EMR

Qualifications

Current and unencumbered RN license in Texas and a Bachelor’s degree in nursing (BSN) is preferred but not required.

Skills

Strong communication skills and the ability to build rapport with patients and families are essential.
