Responsibilities
- Perform patient assessments, admissions, and ongoing care planning
- Coordinate care with physicians and interdisciplinary teams
- Deliver therapeutic, preventative, and rehabilitative nursing care
- Maintain accurate and timely documentation
- Monitor patient progress and update plans of care
- Support patients and families through all stages of care
Requirements
- Active Registered Nurse (RN) license
- Graduate of an accredited nursing program
- Minimum 1 year of clinical experience (ICU, Med-Surg, or Rehab preferred)
- Home Health experience (a plus!)
- Current CPR certification
- Reliable transportation, valid driver’s license, and auto insurance
