Our Immediate Care offers a quick, walk-in option for patients to see a provider without scheduling an appointment, treating patients of all ages for non-life-threatening conditions. Immediate Care is open seven days a week, including evenings, with an average wait time of 15 minutes or less. We always communicate closely with our patients’ primary care physicians.

About The Role

Assist provider as necessary; room patients, take vitals, triage patients, call in scripts, and perform other associated responsibilities.

Responsibilities

• Carry out the nursing process in person, by telephone (telephone triage), and via telehealth modalities.
• Assess and identify patient needs, plan to achieve appropriate outcomes, intervene, and evaluate progress toward goals of care.
• Collaborate to design and implement a plan of care with the interdisciplinary team including Physicians, Physician Assistants, and other health professionals.
• Administer prescribed medications, treatments, and vaccines, perform point-of-care testing, and collect and handle specimens within the scope of practice as determined by the Nurse Practice Act of the appropriate state.
• Educate patients on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s).

Requirements

• Current NH Registered Nursing (RN) license.
• Current BLS/CPR training.
• Prior work experience in a healthcare setting is required.
• Epic experience is a plus.
