Registered Nurse Ambulatory - Nashua Rheumatology - Full Time - 36 Hours - Foundation Medical Partners

Responsibilities

• Absorb provider needs as necessary
• Room patients, take vitals, triage patients, call in scripts
• Carry out the nursing process in person, by telephone, and via telehealth modalities
• Assess and identify patient needs, plan to achieve appropriate outcomes, intervene, and evaluate progress toward goals of care
• Collaborate to design and implement a plan of care with the interdisciplinary team including Physicians, Physician Assistants, and other health professionals
• Administer prescribed medications, treatments, and vaccines, perform point-of-care testing, and collect and handle specimens within the scope of practice as determined by the Nurse Practice Act of the appropriate state
• Educate patients on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s)

Requirements

• Current NH Registered Nursing (RN) licensure
• Nursing degree
• Prior work experience in a healthcare setting
• Epic experience is a plus
