About the role

Assist provider as necessary; room patients, take vitals, triage patients, call in scripts, and other associated responsibilities.

Responsibilities

  • Carry out the nursing process in person, by telephone (telephone triage), and via telehealth modalities
  • Assess and identify patient needs, plan to achieve appropriate outcomes, intervene, and evaluate progress toward goals of care
  • Collaborate to design and implement a plan of care with the interdisciplinary team including Physicians, Physician Assistants, and other health professionals
  • Administer prescribed medications, treatments, and vaccines, perform point-of-care testing, and collect and handle specimens within the scope of practice as determined by the Nurse Practice Act of the appropriate state
  • Educates patient on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s)

Requirements

  • Graduate of an accredited nursing program (ASN required, BSN preferred)
  • Licensed RN in NH, MA, or compact state, with current BLS/CPR certification
  • Prior experience in ambulatory care, anticoagulation management, or chronic care preferred
  • Strong communication and clinical assessment skills
  • Comfortable working independently and collaboratively in a fast-paced setting
  • Knowledge of EMR systems (EPIC preferred) and patient-centered care practices
